By typing my name below in lieu of a signature, I hereby attest that I am the legal parent\guardian of the above-named child(ren) and hereby consent to their participation in the activities of C.R.E.W Winter Middle School Convention which include but are not limited to spiritual activities, sports, crafts, trips, free-play and at specially announced times, water games. I understand that activities may accidentally result in physical injury to my child but nonetheless specifically request that he or she be allowed to participate in those activities.
I agree to give the convention directors permission to act on my behalf according to their best judgment in an emergency requiring medical attention and I agree to take responsibility for the associated expenses. I understand that C.R.E.W Winter Middle School Convention does not carry insurance relative to the activities or for any injury that may occur to the above-named child. I represent that the child is (a) covered by insurance through my own insurance carrier; or (b) that I am personally financially responsible for any and all medical costs incurred as a result of the child's injury.
I release and waive, and further agree to indemnify, hold harmless or reimburse C.R.E.W Winter Middle School Convention, the Coptic Orthodox Diocese of the Southern United States, the individual members, agents, employees and representatives thereof, as well as activity supervisors and volunteers, from and against, any claim which I, any other parent or guardian, any sibling, the above-named child(ren), or any other person, firm or corporation may have or claim to have, known or unknown, directly or indirectly, for any losses, damages or injuries arising out of, during, or in connection with the child(ren)'s participation in the activities (including all forms of transportation) or the rendering of emergency medical procedures or treatment, if any.
