Consent and Waiver



  • By typing my name below, in lieu of a signature, I hereby consent to participation in the activities of Pimenreet Girls Winter Middle School Convention which include but are not limited to spiritual activities, sports, crafts, trips, free-play and at specially announced times, water games. I understand that activities may accidentally result in physical injury but nonetheless specifically request to participate in those activities.  

    I agree to give the convention directors permission to act on my behalf according to their best judgment in an emergency requiring medical attention and I agree to take responsibility for the associated expenses. I understand that Pimenreet Girls Winter Middle School Convention does not carry insurance relative to the activities or for any injury that may occur to me. I represent that I am (a) covered by insurance through my own insurance carrier; or (b) that I am personally financially responsible for any and all medical costs incurred as a result of any injury.

    I release and waive, and further agree to indemnify, hold harmless or reimburse Pimenreet Girls Winter Middle School Convention, the Coptic Orthodox Diocese of the Southern United States, the individual members, agents, employees and representatives thereof, as well as activity supervisors and volunteers, from and against, any claim which I, any other parent or guardian, any sibling, or any other person, firm or corporation may have or claim to have, known or unknown, directly or indirectly, for any losses, damages or injuries arising out of, during, or in connection with the participation in the activities (including all forms of transportation) or the rendering of emergency medical procedures or treatment, if any.
